This app is very easy to use and perfect for our needs. We are a flower shop and were able to add required fields to the product page to collect a card message and recipient's phone number. I decided I wanted to add the card message to the customer's confirmation email so the customer could review the card message they inputted. Customer service got back to me right away with very detailed instructions, including screen shots, on how to add the code for this. I highly recommend this app!

So far this order form app has been great! Integrates well with my site and the customer support is fantastic. Would absolutely recommend.

doesn't work at all. i wish it did. you can set everything up but when you preview it, the app doesn't work. waste of 3$ edit: it works but you have to change your theme settings. took me a while to figure out
