Excellent app with great customer service! Works as it should straight "out of the box". Easy to set up and it's increased my custom order inquiries greatly since setting up. I had an issue with customers being able to chose past dates from the date/time picker and I emailed customer support to see if that could be fixed and they made it work properly almost immediately! I use this form app in this site as well as several other stores I manage. Highly recommend.
