Not fit for purpose. Installed it and seemed to be working great - but won't let me delete fields now from the form. Worse still, everytime I rename a field, it adds it as an additional one too. Tried re-installing and encountered all sorts of problems there too that Shopify Support had to solve. Reached out to the app developer for support but not heard anything back after 24 hours. Shame, as the app has the functionality I need - it just doesn't work properly.

App works well. Does what we need it to do with easy customization. Submitted a few feature requests to support, that would make it a 5 star.
