I have been using this contact form builder for years now. It's fairly easy to set up and customize options. It has a clean look and is easy to add to website pages with the automatic or manual option. Highly recommend!

Excellent Support & Reliable App We’ve been using Nerdy Form Builder for our store and it has worked very well. Recently, we faced an issue with the sender name not displaying correctly when using a custom SMTP. I reached out to Anton (the founder), and he provided outstanding support. He quickly made updates to the app so that our emails now display the correct sender name, which is very important for our branding and customer trust. The app is simple, effective, and the support is fast and personal. Highly recommended for anyone looking for a dependable form builder on Shopify.
