Perfect for what we needed to do! It has conditions and and you can really customize your forms in several ways! I had a little issue with some conditons but support was quick at fixing it!

Pify Form Builder is user-friendly and makes it incredibly easy to create and edit forms for your Shopify store. The app also also allows you to manage responses. Highly recommended.

Very pleased with this app for our website. As a design company, we wanted to gather as much information from our clients as possible, and out of all the form apps we found, this one was the best. Really great for a free form. I wish I could customize the width of the form because it looks super skinny on my page, but besides that, its great
