We're incredibly grateful for this application. The look and style is perfect, and it's free. Lovely app and while it needed a little setting up, it was easy. The instructions were clear and the app works perfectly. Thank you.

Easy to set up and customize the question and responses. Doesn't seem to be any options to change appearance and do multiple questions, but suited my purposes perfectly.
