Formly

Formly is a Shopify App that allows ecommerce merchants to collect custom product options and save them in their orders. With Product Options and Terms, merchants can add custom fields to their product pages, enabling customers to input text, select from dropdowns, and check checkboxes before making a purchase. The responses are automatically saved as line item properties in the Shopify orders, streamlining the process without the need for manual work. The app is compatible with both Add to Cart and Buy It Now buttons, and merchants have the flexibility to assign fields to all products, specific products, or collections.
