Formly

Formly is a Shopify App that allows ecommerce merchants to collect custom product options and save them in their orders. With Product Options and Terms, merchants can add custom fields to their product pages, enabling customers to input text, make selections from dropdown menus, and check checkboxes before adding items to their cart. The app automatically saves customer responses as line item properties in the Shopify orders, eliminating the need for manual work. Formly works seamlessly with both Add to Cart and Buy It Now buttons, and merchants have the flexibility to assign fields to all products, specific products, or collections.
