Professional Form Manager is a Shopify App that allows ecommerce merchants to create custom forms on their storefront with file uploads and receive submissions by email. The app can fill a copy of the merchant's own PDF with each submission, or automatically generate a PDF for forms without one. It offers features such as review submissions in the admin panel, masking sensitive answers, exporting to CSV, and built-in spam checks.
