Great app and even greater and really fast support! That's how it should be! As far as I know the only app which can help you to get additional fields into the cart without having the shopify plus plan. 100% Reccomendation!

Does what it says, does it well and inexpensively, to boot! The dev has been super helpful and responsive in guiding me through integrating the fields into pages in my theme!

Flavio gives all suport we need anytime. It helped us to insert a client's ID field before the checkout. Very helpful app! Thanks

This app is very easy to use, and is perfect for gathering information necessary to complete a sale. Customer service is outstanding. The staff went way out of their way to develop a special "conditional" form for our store. I recommend this app for any store that needs a form.
