It is a basic app, but it works well. If you don't need really complex and animated form i think that it is perfect

I'm using this app to create a form. Required some customization on the existing form fields, but that was not part of the app. Developer created fields for me and modified the app as per requirements. Support is quick and respond back positively. Amazing app and great customer support. The feature are amazing and easy to use.
