I recently started using SK: Advanced Form Builder. I'm finding it very easy to use. It has all the features I need, including check boxes, custom fields, and customizations. But the best part is the great customer support.

Recently setup to be used as a Returns Form and works great with easy implementation into both the shopify store and our current internal RMA process. Considering it is Free I can happily say it is the ideal option for Small to Medium Business where a quick form is all you need.

Form building using SK- Form builder is not just quite easy but also very efficiently craft the form and get data from your user in a very effective manner. I highly appreciate SK- Form builder for any Shopify store.

Great customer service when implementing the form. Needed to change the fonts and they gave me the full custom CSS to do it. Thanks
