I've used a few different form apps on Shopify and this is the best one I've found. It's super easy to use and offers several different integrations w/ external platforms (Google Sheets, Klayvio, Slack, etc). If you want a simple, easy form builder for your site, this is the one!

SK's Forms is simple and easy to use and perfectly meets my requirements. I have used other form apps before, but the experience is very poor, such as not being able to receive email notifications, not being able to open the app, etc. But SK's form avoids these problems perfectly. Highly recommended!
