Hubify Product Custom Options
Hubify Product Custom Options allows merchants to create multiple options for products and customers to submit additional details with their orders. Customers can easily use the custom product options during checkout, enhancing their shopping experience.

It is relatively easy to use. Was able to get it set up quickly. However, Checkboxes weren't transferring to the cart at all, and to my horror, NONE of the fields were captured in the order. So my customer spent time personalizing her product only to find that I had none of the information she input! Had to scramble and find another app to use. Also allowed customer to add to cart without filling in anything, even though I had selected required.
