Zoho Survey
The app seems to have issues with functionality as users reported that nothing worked despite their attempts. Additionally, there are complaints about poor customer support with unanswered emails. Users expressed frustration with the app not meeting their needs and the lack of assistance from the support team.
This was almost exactly what I needed, except nothing worked. Did a few tests, but nothing. Wish it worked.

can't send the survey by the customer list, contact their support without any reply. till now still haven't replied my email,
Hi, Sorry to hear this. We have already replied to your email. Reply was sent your store registered email address. After some Shopify update customer build actions (Send survey) is not showing. This has to be fixed by Shopify team. We have already contacted and reported the issue. We will update you once we get reply from Shopify support.
