Was looking to do wholesale and needed some sort of a form. I found SureCust ‑ Customer Forms, the forms are really professional looking and can be customized to what you need.Only took a few minutes to work it out.Needed support and " WOW " very responsive and polite. Recommend yes and support is worth more than five stars,great app and amazing support.

Great product, direct integration with shopify and directly integrates with account verification emails sent through shopify. Perfect for wholesale stores that want to eliminate the manual collection of business info, and I love that you can automatically approve accounts, that makes the user experience 10 times better!

Needed a simple to create/use form app that created a nice looking wholesale customer registration form for our website. This did the job perfectly. Until we realised that the County(State) hard coded options were incorrect. Their support team jumped straight on it and within a few hours had fixed the issue and updated the entire UK county options. Very good app and very good support. Recommend.

This is exactly what I have been looking for! I have a wholesale side of my shop for which I needed a better registration system. I was using a paid for app but as a very small business just starting out who only needed one basic custom form, I hated paying such a fee while I still had to use a workaround on my site to make the user flow not terrible(not great either). SureCust fixed all of that for me! Within an hour of installing the app, I had a new workflow for my wholesale applicant that is streamlined and simple. Not to mention as a sole proprietor business owner with plenty to do to run the business, I was able to cut my administration flow down to two steps. It does everything I need and is extremely fairly priced as their model is based on the number of custom forms you need. As with other reviews, their support is polite, fast, and responsive. I had a couple of custom requests that they implemented immediately. As a UX designer by trade, I can't help but make improvement suggestions, to which the support team is delighted and eager to add to the app.
