This app is just outstanding. Really well thought out, easy to use, lots of flexibility. We were able to create a detailed registration form to replace Shopify's native one. Form submission results get sent to an email address, get stored in the app itself and can also directly populate a customer record. Really impressed with the feature set and the designers were incredibly helpful in assisting with some issues and customizations. As good as it gets!

Great value and return with excellent response time and impact. We have a very complex input form for custom orders and this tool handles the inputs nicely.
