Before I state why - if you need to install a quick order form to your webstore - INSTALL THIS APP. I recently contacted the developers (and Karina in particular) to request some assistance with installing the app on our dedicated B2B pages under Shopify's new Markets option. They were unwavering in their support to give me EXACTLY what I needed, in terms of showing it to certain customers whilst hiding it from others - not only that but they assisted me with styling the form also. Initially there was a bit of back and forth as to whether or not they could help me, but after having a decent conversation as to what we wanted, they DELIVERED.
