Even though we're only using the free version, it has nice amount of features. The app is easy to use and has worked very well for us.

Excellent app, works great. I use it for a offer form where clients can submit for an offer. So far I didn't need any support, so the app is very intuative.

Thank you so much for this app! Since Shopify does not provide any preorder option without the customer having to pay in advance this app was the only good alternative that we found after searching for a long time. The Pro Plan is totally worth it. The only thing we are missing (or not finding) is a mass change function for changing multiple forms at the same time. And maybe a better translation for German but with a bit creativity we are able to understand the language quite good! xD The support is also really fast in helping out when we are struggling with something.

I wanted to build a form for my customers to upload pictures for potential commissions, and add information that I would be asking them in the initial response anyway, and Form Builder with File Upload was perfect. Easily customizable too, so it doesn't stick out like a sore thumb on my page, even with my limited back-end design experience. Two non-sore thumbs up!

This app is by far the best app that has helped us with our Wholesale Registration form. It is quick and easy, beautiful templates and design. As well as very quick and fast technical support.

App works really well . Support is good too . easy to setup and configure . we needed an app which could allow us to make a option in form where customer can upload files .we found this app and it did the job for us .

A very extensive application, especially in the paid version, has great possibilities. I am using the free version and it is enough for me. Support usually solves most problems within 30 minutes.

There is no fully functioning 7 day trial like there is with most apps, so you have to pay a month subscription to see if it will meet requirements, which in my case it didn't.
