Hulk Contact Form Builder

Super easy to install!! The support they offer is fantastic. The options you have to create a contact form is amazing and this is exactly what we were looking for.

The app is exactly what I needed. It works perfectly well and the team is extremely helpful and fast in responding. I highly recommend it!

Easy form builder? Yes please!! Super easy to use, love it! HulkApps does it again. I use so many of their apps!

We initially began using Form Builder to create our CONTACT US and WHOLESALER ENQUIRIES form - both of which you can see linked to our front page (www.samana.com.au). We really like HULK's Mailchimp integration that kicks in when new Wholesalers apply. It's also great when we can retrieve from the HULK database all of those people who have submitted applications, along with their responses. The "Create Account" option on the home page of our Theme does not have a Captcha. With prompt assistance from the HULK developers, we replaced the existing form with one we designed using the Form Builder and HULK's Captcha component. We had a few hiccups but it was all completed in a timely manner. Thanks HULK for your support!

The design is clean and minimalist and works like a charm. Support is also available when you need them, which is amazing.

I used Form Builder to create a Wholesale Application form for our store. It works flawlessly. Was really straightforward setting up all the form entry fields and text, and most important has reCAPTCHA feature which sad to say is now needed for every form on any website. I highly recommend this app based on my limited usage. I am using it in Free mode as that is all i need. I haven't looked at Pro mode. A side note. I wish Shopify had reCAPTCHA for the "register" page and other form pages in a theme. It's crazy that Shopify does not build reCAPTCHA into Liquid so Themes can use it to add to forms. Come on Shopify! You can do it!

We use this form to generate quote requests for our product. It works like a charm! When we have had minor issues, the tech support team has been responsive and capable. I would highly recommend.

I used this app to create a form that customers can use to sell products on my store - its worked great and we are really happy with the way you can fully customise the form to suit your needs. Needed just a bit of help to complete set up after watching a set up video and customer service response was very fast and sorted it straight away - thank you!

I tried every form app out there and this is by the far the best! The customer service is amazing too if you ever have a question or need something tweaked, as well. Highly recommend.

If there were more stars we would give Hulk company all of them. This is an exceptional company that has one of the best customer service programs on the Shopify App Store. They are quick to respond; very helpful and are always there. We are not app savvy but their app is so easy to use ... user friendly ... anyone regardless of your level of expertise won't have any problems. But if you need help, they have developed one of the best resource libraries in the Shopify store. We are so pleased to have found Hulk and know you will also feel the same way.

App stopped working. Can no longer access any of our webforms. Tried going in today and it is loading super slow (keep in mind no other app is performing like this). All I see is an icon of a man with a jet pack (guessing their loading icon) but I cannot access any of the webforms. Its as if we are locked out. I liked how the app worked and functioned but if I cannot access the app or my webforms its pointless for me. I did not enjoy that I could not access my webforms, that was a disappointment......

Overall nice and intuitive UI, and easy to use. One issue I have is my form data "Address" fields aren't properly getting piped into MailChimp. It does seem like this is an issue other form building apps have too, so might be a MailChimp thing. General workaround is instead of using out-of-box Address field, create your own separate text fields for "address", "city", "state", and "zip" and then map over manually to MailChimp. Finally, I've been in contact with Bhavya and they've been really helpful in getting another one of my issues resolved.
