Good app, but needs work. We needed a multi-step form, 2 conditions and a few file uploads. The form kept breaking, any select/checkbox items would all get mixed up and break the logic, and the logic itself would break when using the conditions to hide and show fields. The required logic for all other file upload files except for the 1st file upload was ignored, thus the required info was not provided. The step-nav itself would allow for forward clicks, completely bypassing any required fields within the steps. When flagging with support; The assistance to block forward clicking via the nav was not possible (our dev found a solution to our problem and has passed this info on to the support/dev team of the app). The most pressing issue regarding the file upload requirement was flagged, and after a week of following up on the ticket, we have received no feedback or any update on this. We have chosen to uninstall this app and find an alternative.

Takes a little bit of figuring out, but it's overall very easy to use and integrate and has lots of features. Support is also there when needed!
