We researched quite a few form builder apps to add to our Shopify store and found SK Form Builder to be the best so far. It offers a vast range of customizable options and also has the feature of data collection and export. We have been using this app for over six months now. They have been very helpful and supportive in all our customization requests, and I recommend using this app. The paid version is also quite affordable.

One of the most useful, flexible, and easy to use Form app for Shopify. We have been using it for a long time now for all of our website registrations and other submissions. The highlight for me is being able to embed it using a simple code which makes it super easy to use via meta-fields. 100% recommended.

Simple to install and use. Feature rich and highly customisable making it easy to match theme style. Can't comment on support as not encountered an problem requiring their help. As far as form builders go its one of the best we have used.
