The form builder is a breeze to use. The ability to use conditional logic to show or hide various items on the form based on user input is also great, as is multi-step form options. The form has so many options for customization built in, as well as the ability to use custom CSS, so it can fit into nearly any site design. And, if you need help, the developer is also very responsive and quick to reply.

Do not INstall this Stupid APP THIS DOESNT WORK AT ALL!
Hi, We're sorry for the bad experience the app gave you! Perhaps the installation and usage time is too short (only about 20 minutes) so you don't really understand how to set up the app and maybe you haven't looked through the app's user manual. This is the only form application that can send emails to other systems (e.g. CRM) where the sender is the email address from the email form field. A key thing for our CRM system. I highly recommend this app and am very happy with the fast responsive support.

I highly recommend this app because once I set it up last year, it has worked so smoothly that I forgot about it. I recently made some edits to my theme that caused an issue with the app. I contacted support and they fixed it right away. Thank you for the quick fix!

Very easy to create, no developer needed. Great support team as well. The basic form is free and you can also upgrade to include Departments, Attached files, Auto-response, Thank you page redirect, an Info box, etc. Great price too.

Does not work at all. After installation the whole store froze so we had to uninstall and switch to a different plugin
Hi, We are very sorry for the problem with our app! Since this is a theme-related application, there may have been some minor conflict with your theme. Usually when having this problem customers will contact us to get support and we will fix it quickly. Unfortunately for us, after installing the app only about 2-3 minutes, you uninstalled the app without contacting us. Your store is not officially live yet, we hope you give us the opportunity to help you fix the problem and create the perfect contact forms for your store. Kind regards,
