easy to setup, but it has a few problems. (1) after submission, the CLOSE button does not work. (2) if your shop is xyz.shopify.com but X-Home is your actual website, the confirmation email to customer shows xyz, not X-Home. (3) Order Number should be a required field (4) Reason for Withdrawal should be a required field (5) Merchant doesn't get a confirmation email notification although an email has been specified for that purpose - you only can check submissions via the app. Hope these can be fixed, otherwise I'd say this is still very immature.
