We are using this app for new wholesale customer applications, approval and customer creation. The custom forms are flexible and highly configurable. I needed some form fields copied to the customer account fields when the customer is created, and the developers were very responsive to my needs and took care of it. Very impressive customer service!

This app forces me to pay for its use. There is a free plan, and I don’t need the paid plan for the time being, but it still requires me to pay. This is very unfriendly. No matter what I click, I will be redirected to the payment page. If you don’t want others to use it for free, you can cancel the free plan.

This app has made it easily to solve authenticating possibly fraudulent orders. We use their email sign up form, but converted to a verification form and it has been perfect. Very easy to use and the best looking of all the custom form apps we tried. Highly recommend.

This app is exactly what I needed, and functions just as well (if not better) than all the other more expensive apps I’ve tried. I had a lot of trouble setting up my email notifications to my customers with the app for some reason, but customer service did it for me so fast!
