Update to a previous review. Switching to a new theme and needed a form move to a new page on that theme and formatted for it. Customer service got it done within 24 hours. just amazing and always here to help when i need it! Definitely worth the cost. Customer service was amazing. I needed a job application built and Chirag was on it right away within a few hours and helped me create it just as I needed it. Super quick to respond when I had a question and explained how things worked for me, even despite the time difference. I would highly recommend this app if you are needing any kind of forms created!!

Very happy with the quick customer service, I worked with Chirag Patel and he helped me get the formatting correct. The form customization is very good, with logic flows too. I only had trouble because the form stretched across the full width and I wanted it to match the rest of my website that wasn't full width. Chirag was quick and efficient to resolve this!

Excellent support and quick responses from Chirag! He knows exactly what you need and was very helpful in getting our contact form up and running. The best features of this app are the ability to enable reCAPTCHA and multiple email recipients based on selections. Overall, highly recommended!

We installed this app in the hopes that we could some how figure out a way to install a contact form on our product page to intake consultations for a specific product of ours. We had a vision, but we had no idea how we were going to enact it! Chirag, a representative from the app, took our vague idea and made it come to life! With only one email containing instructions, he did exactly what we were looking for to the T. I would recommend this app and its representatives to anyone looking to incorporate any kind of contact form in any way on their Shopify store!

Would like to keep it simple and to the point. Their app is very clear and simple to use which is why I chose this company over others. These guys are amazing at what they do and the support they have provided us with our business has been outstanding and they are very fast at getting things done. We needed a very complex form built and they completed it without hassle and explained everything along the way. They went even further and provided us with support beyond the form and website too which just shows how genuine these guys are. I would definitely not bother looking elsewhere if you need a form implementing whether its simple or complex these are your guys their knowledge with Shopify is excellent.. These forms work great! Highly customizable, clean, and responsive. When I needed help setting up the forms, their support team replied lightning-fast via online chat and helped solve a massive task within minutes ( That was honestly amazing!) I would like to give kudos to Chirag Patel for being friendly, patient, and super-helpful. We have tried many other forms with more reviews and users but none of them had the same functionality or the same level of customer support. Absolutely amazing! Thank you, team!

It's really a great contact form app and I used this app as Request a call back form on homepage & product pages and it works fine for me. I have been using this app since past one year and so far very happy with app features and quick development support. They upgraded app recently and we saw some new updates already in backend. We saw the problem with Google ads couple of day ago but very happy that its resolved quickly by their developer. I highly recommend this app and confirm that this app is 100% SAFE TO USE!!

We used this app. But then we realized it redirects users to malware / virus sites. it was hard to notice as it only happens once per device. DO NOT USE THIS APP

Amazing app, which helps us to create a form for B2B and customer satisfaction. It's very easy to instal and use, but If you have some basic knowledge of HTML you can use this app to full potential.
