This app seemed to be one of the simplest of the options as I was reading through descriptions, and I was right in my choice. I got the form built with no issues, I ran into a small issue with placement on my page, chatted with support (Vedran) and the issue was fixed within a minute or two...turns out, it was an error on my part.
