Great app, user-friendly and quick customer service responses. They guided me through adding a checkbox to my form when I wasn't sure how to do it.

UPDATE from ~6 months in: Still 5/5 stars. I've reached out to support for the odd question here and there, and every time I've gotten a response within *minutes*. And it's not just an auto-responder. It's a knowledgable, helpful person every time. Can't say enough good things about this app. Original review: I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need). The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections. 5/5 stars!

You can customize the form style and information on your own. The features are very comprehensive, and the setup is straightforward and easy to understand. Customer service representative Smiljan responds very quickly. Highly recommended!

I initially had trouble connecting to Zapier (confusion on my part), but tech support was outstanding and clarified the issue very quickly! Tarik in particular was incredibly helpful. I quickly had my new form (a "Get Free Sample" form) up and running live and connected to Zapier for additional automation flow (create new order in my shipping software). The form builder itself is intuitive and flexible. I particularly like the ability to create new Shopify customers when the form is submitted (optional feature you can toggle on/off) and to also limit the form submission to one per email address (can also toggle on/off). There are lots of additional features that I am looking forward to using. I've tested numerous form build apps and Hulk Form Builder is a great product and the tech support is excellent! I can highly recommend this app with confidence.

This is a great form builder. I like the dependencies and features that they offer. This form embeds well on our website and doesn't cause any issues with google/SEO. That being said, there is one thing that I would really like to be made possible, hopefully ASAP. The email notifications for the form submissions come in a HUGE chain that seems to reset maybe twice a day (if that). Since we use Hulk for our contact form, there's anywhere from 3 to 40 forms in a single email chain. All different customers with different needs. This makes it very difficult to organize, forward and respond to inquiries - and VERY easy to miss contact forms. We do use one other form service for our website, Monday.com. Many of our forms are hosted on Monday.com, but there are issues with Monday.com forms messing with google/SEO so we use Hulk for any forms that NEED to be embedded on our website.

Terrible app, so laggy and inaccurate data! Once I chose a form it started to be so laggy and I couldn't access the customer submissions. Also, I paid extra fees to get my data through integrated Google sheet, it's delayed for days to be synced. I started to search for an alternative already.
