The form we built using this app works well, except that it is incompatible with our email (Office 365), so we are unable to take advantage of having email confirmations look like they are coming from our own business. We are looking into work-arounds. Hulk Apps tech support was very responsive and friendly; they just weren't able to provide a workable solution.

Really great app, I am on the free plan (at the moment) but will probably upgrade when the time is right. I had a specific coding requirement and they fixed this for me very quickly with friendly service. I can't complain about anything! Thanks, Guy

Works ok but lots of downtime. I'm currently down on 3 websites now and can't offer support (over 8 hours now) and no ETA. Just getting a "formbuilder.hulkapps.com refused to connect" each time I try to load the app in any of the stores. Also a pita to get support with them, you have to create an account type of thing. 1 star for that. Update as of a week later* Still having issues with the apps, removed them to fix it and when I try to reinstall I'm actually seeing my old forms pop up for a second then I get an error from Shopify saying the page doesn't exist. So as of now I'm not only down a bit, but completely as of now for my forms.

I installed the product option form first and after continuously amazed by their service in helping me fixing all the issues - I decided to also replace my previous contact form with Hulk form builder. I enjoyed working with them and they are the best in terms of timely responding and solving your issues.

Easy to use . East to manage. Replaced old HTML table in 2 weeks. Forms are kind of old fashion but it did the job. Thanks.

We are using this for multiple sign up forms and image upload function via embedded forms and it works really well. The klaviyo integration is great and the support staff (Amit) are really quick and helpful.

My website is in French and it is impossible to write french without apostrophes (single quote character). It is also almost impossible to write English without single quotes. This app does not support single quotes in labels! When a developer does not bother to handle special characters in their UI, it makes you wonder about what other shortcuts they have taken. It is a shame as I like the app, on paper. It is well thought out and functionally rich. Uninstalling reluctantly

I would like to clarify that our coding structure uses inverted comma/single quotes for building the functionality. If a user adds any value with a single quote in the form label then it will conflict with our app code and break the functionality. Thank you for your understanding! I am a new user of Shopify for my small business and I have a design background but no tech background, so good customer service is especially important when I'm connecting apps to my site since I do not have much coding knowledge. The team at HulkApps has gone above and beyond to help me make adjustments, embed my apps, and has made sure that I am completely satisfied, at no extra charge! I used this app to create a suggestion box at the end of my blogs on my site to get feedback and suggestions for future posts! I love the way it looks and that the suggestions go straight to my email. HulkApps is my go-to for any apps I want to add to my site because of their fast and efficient communication!

This app is the solution to all of our complicated form building challenges. The support team has been extremely helpful and quick to respond to any special requests we have. The premium version is 100% worth it—don't think twice.

I am currently using the Free Version, A perfect app to create forms. Pick and drop sections, easy to Use. Spontaneous Customer Support. Recommended if you are looking for Customer feedback/ query forms.

App is nice but it doesn't have password field type in element. unable to create registration form with free version.

I needed a simple account registration form to set up customers in Shopify. This looked perfect but wouldn't actually set up customers. I requested support twice and after about 10 days I could see they were testing, but another two weeks later the issue was not resolved and I still have not received any communication from them regarding the problem or a possible solution. So I found a better app elsewhere.
