I used this app to create a "Get Involved" form. It's free, and it works great. My favourite feature is that you can have some questions only appear on the form if the visitor selects a specific option from a drop down menu. Also, you can have an automatic response sent to the customer from your email address when they fill out the form - so they know that the form went through. No complaints here - great options for a free app.

I use Form Builder for so many things now, it's such an awesome way to get information from my team! The customer service is top notch, the app works great, and it's free... Can't go wrong!

This app could be perfect as it is ideal for what I need, however it's temperamental when working on Safari when I need to edit a form and get onto the app. One day it works then others says I need to look into my cookies setting. Can be quite annoying!

It's so easy to build forms and utilize them on landing pages. If you're intuitive the basic and free features are great, if you want to get more in depth, they have a lot of great perks and add ons!

Love this app! There are other form builder out there but they all have that "branding" that you have to pay for to remove, Hulk's form builder doesn't and it really looks smooth and professional! We love Form Builder! It's simple to set up new forms, they're attractive looking, and it has all the functionality we need. We currently use Form Builder Pro to manage our return request form, and our wholesale application, and it does exactly what we need!

Simple and easy-to-configure form builder (far better than Shopify's built-in form builder), but advanced changes require a working knowledge of JQuery/CSS. Support is helpful and friendly but not always terribly timely. The perils of a small operation. Still, I would recommend the app.

NOT FREE Listed and promoted as a free app. Once Downloaded they ask for you to upgrade to complete the form. Crappy Crappy stuff guys. we are tired of fake sale and advertising pitches WHERE ARE THE HONEST BUSINESS NOW DAYS!

Best Shopify Form Builder with Conditions and Hidden Field. I bumped into form builder app when I was looking for custom work for my feast box menu. I wanted some fields to appear only if a condition was true and then I used their Advanced CSS block to get some details about my menu items within a hidden field. Tip for consumers: once you try it, you will be impressed on how many integrations they have!!!!

I use the HULK Free Form Builder app to build forms on my Shopify website www.westermanbags.com * A customer feedback form, mostly for items which were purchased outside of the Shopify store. (Regular purchases get reviews through the Shopify review app. ) * An order form for purchases which need to be paid with with a giftvoucher. This way I can skip the expensive Shopify upgrade and still offer giftvouchers, albeit a little more work in the setup and maintenance. The final integration of the form in webshop was done for me, I am very satisfied with this FREE Hulk app.

Need a save button on your app. And when I click on an element that is "required" for the form, like a customer's name, that shouldn't mean I'm required to fill out the form to publish it. Glitchy, I guess? Lost 2 forms and wasted hours of my time. Ridiculous. I will say it was fairly easy to use and had all the elements I needed to build a great registration form, but if you can't save or publish it - what's the point? Hope to see you correct these issues. Disappointed.
