Really great value for this app. Does just what I need it to. Hulk has great support, they answer all of my questions and have helped me install Apps. I would take a look at the other apps they offer too.

Love this! Haven't found anything in it that would make me NOT recommend it. A few tweaks and options that should be free, sure, but this app is super easy and I love it.

We use the form builder app on many pages on the store. It is very useful for B2B registration, custom work forms and much more. The custom CSS block makes all the difference for us. We can use it to share more information with our customer based on the options they select. The support team does a very good job too.

I had a great experience working with them. I asked where I wanted my custom form to show on my website, and they did the programming in no time!! I now signed up for their paid App. I highly recommend this App.

Your team developed our primary form with about 30 questions and uploads for our application. Suggestions for improvement; A) After selecting a file for 'upload' display the name of the file selected, last updated date/time, file size; B) In the xlsx stored at HulkCode after SUBMIT, add a column for Form Start date/time; and change last column name to Form Submit End and store the date/time rather than just the date; C) allow short column naming by your customers in this xlsx rather than text of the question as the column header; D) Because data submitted is NOT password protected on your server allow DIRECT EMAIL to attach all data directly to destination email address to avoid accidental discovery of PII, HIPAA, GDPR protected data that could be in any SUBMITted file and result in significant financial penalties to your customers and Hulk Code. Your development team is to be commended for their talent in completing this task. www.mysqlservertuning.com

We initially began using Form Builder to create our CONTACT US and WHOLESALER ENQUIRIES form - both of which you can see linked to our front page (www.samana.com.au). We really like HULK's Mailchimp integration that kicks in when new Wholesalers apply. It's also great when we can retrieve from the HULK database all of those people who have submitted applications, along with their responses. The "Create Account" option on the home page of our Theme does not have a Captcha. With prompt assistance from the HULK developers, we replaced the existing form with one we designed using the Form Builder and HULK's Captcha component. We had a few hiccups but it was all completed in a timely manner. Thanks HULK for your support!

I used Form Builder to create a Wholesale Application form for our store. It works flawlessly. Was really straightforward setting up all the form entry fields and text, and most important has reCAPTCHA feature which sad to say is now needed for every form on any website. I highly recommend this app based on my limited usage. I am using it in Free mode as that is all i need. I haven't looked at Pro mode. A side note. I wish Shopify had reCAPTCHA for the "register" page and other form pages in a theme. It's crazy that Shopify does not build reCAPTCHA into Liquid so Themes can use it to add to forms. Come on Shopify! You can do it!
