Don't waste your time. This app is glitchy at best and horrific at worst. It doesn't work at all the way you envision. They need some better engineers.

Great customer service, great form builder. I love that this allows you to have unlimited form fields, intro before and after the form. I also appreciate that you and customize the look of the form to sort of match your site styling. The styling also offers hundreds of free fonts (Google Fonts), which is fantastic. I'm giving 4 stars because I did have a bit of trouble in that I was creating a work application for a client, and repeated some of the same form field names, and it resulted in a buggy email form that left off so many form fields. So I wish that would be in the instructions before you begin working on the form. HOWEVER!!! The app developers were so quick to examine the form to figure out what the bug was when I brought it to their attention, and they let me know how to fix it quickly. The form performs beautifully now, and looks great! The client is very, very happy with it. I appreciate when developers respond quickly, are courteous and explain solutions in an easy, digestible way. A+ on the developer communications and quick solutions. 5 stars on the customer service, appearance, and unlimited fields. Would definitely suggest adding in the detail to not repeat form field names. I would give you 4.75 stars if that was an option!

This is a great form app for Shopify! Easy to use and very intuitive. You can create sophisticated forms with drop downs, radio buttons, and multiple choice. Easy autoresponder integration. Easy to adjust look and feel. I can't believe its free. Thank you HulkApps! Nick at JohnsCrazySocks.com

Exactly what I needed. Very user friendly. I need an form that could collect receipts. Forms online with this functionality charge because they know it's high in demand. I am glad HulkApps made this feature free. The app itself tends to freeze for a few seconds and I'm not sure what's causing it. However, there is no issue with the app or how it works while live on the website. I am very please with the functionality and user friendliness of this app.
We are glad you gave us a shot. We agree that we need to improve our app constantly to help you. We promise to get better with time. :)

Works beautifully! I needed to add a lot of custom fields to the form. I have tested it a few times and edited it over and over, which was really easy. Just a little slow to load, but I have some text on the page also which loads first, so it is fine!

GUI is nice and integrates nicely with Shopify.. but the app is a bit buggy... the file and image upload forms failed to capture the image or file data in the form and wouldn't forward to the capture point aka the email inbox when the form was completed and submitted... opened ticket to try to resolve and exploring other form options

I review very few apps on Shopify, but this one impressed me. We have used it for 3 forms, which took no time to customize. I have to say though, the best is that it's free without slapping their logo on our page. Thanks HulkCode

Highly recommended. Best app we have used on shopify. We required a very specific form and it was incredibly simple to create with more than enough features. Support got back to us within hours. Definitely trying their other apps and themes out.
