Another great app from HulkApps! Form Builder is easy to use and gets the job done! You can spin new forms up very quickly!

Overall nice and intuitive UI, and easy to use. One issue I have is my form data "Address" fields aren't properly getting piped into MailChimp. It does seem like this is an issue other form building apps have too, so might be a MailChimp thing. General workaround is instead of using out-of-box Address field, create your own separate text fields for "address", "city", "state", and "zip" and then map over manually to MailChimp. Finally, I've been in contact with Bhavya and they've been really helpful in getting another one of my issues resolved.

For some reason our mail servers don't like emails generated by this form app. Developer says to create a Gmail or Yahoo account to solve the problem, but what reputable business does that? That's not a solution... Does this app actually work with regular mail servers? Are there workarounds to this issue? Any information greatly appreciated.

Easy to set up a form. But not useful at all because you don't receive any submitted forms. And that is not very nice.

This is the first review I have ever written, but Hulk Apps have consistently come through for me. Form Builder is simple and intuitive and I like that there is a free option, but can upgrade for more features if desired. I needed more support for Volume & Tiered Discounts, but the Support Crew worked with me until I got exactly what I wanted - for free. Quality product, great service.

What great app for forms! Even the free version is loaded with features, so I feel like the product as a whole is solid. Definitely worth installing if you've ever tried hard-coding a form before.

We use this app to allow our customers to submit warranty request forms. The ability to upload pictures is great. It surpasses our needs and there are no cons to speak of.

We love Form Builder! It's simple to set up new forms, they're attractive looking, and it has all the functionality we need. We currently use Form Builder Pro to manage our return request form, and our wholesale application, and it does exactly what we need!

Simple and easy-to-configure form builder (far better than Shopify's built-in form builder), but advanced changes require a working knowledge of JQuery/CSS. Support is helpful and friendly but not always terribly timely. The perils of a small operation. Still, I would recommend the app.

I use the HULK Free Form Builder app to build forms on my Shopify website www.westermanbags.com * A customer feedback form, mostly for items which were purchased outside of the Shopify store. (Regular purchases get reviews through the Shopify review app. ) * An order form for purchases which need to be paid with with a giftvoucher. This way I can skip the expensive Shopify upgrade and still offer giftvouchers, albeit a little more work in the setup and maintenance. The final integration of the form in webshop was done for me, I am very satisfied with this FREE Hulk app.

Need a save button on your app. And when I click on an element that is "required" for the form, like a customer's name, that shouldn't mean I'm required to fill out the form to publish it. Glitchy, I guess? Lost 2 forms and wasted hours of my time. Ridiculous. I will say it was fairly easy to use and had all the elements I needed to build a great registration form, but if you can't save or publish it - what's the point? Hope to see you correct these issues. Disappointed.

Empty/unused elements will not let you save your form. It's better if you allow us to create a form on your behalf. We don't want to waste your time at all. Thank you
