Form Builder is easy to use and has always worked well. We recently had an issue arise with our forms not redirecting to the Thank you page after submission, and the customer service team fixed the issue immediately. Good, reliable product.

I love this app , they offer many type of forms and features. Even they do custom things as per your requirement just for your store. Must recommended app. They also reply quick and provides good support.

Overall, a good app! Serves our needs for limiting access to verified wholesale customers. It allows us to approve or deny all new customer registrations. Support has been very helpful, too.

Form builder has been great for my new wholesale store. The form was very easy to create and customize. Rishav was very helpful and clearly explained to me how things worked. I highly recommend this app.

I rarely leave reviews but we've been using this app for a couple years now and it's been great! Very easy to set-up forms, update registration forms and when we've needed support their turn around is very fast! I highly recommend this app!

It's a pretty good app. You get a lot of customization features, and the conditional logic is the best way to handle customer inquiries. The support is relatively responsive and they can help with all of your needs. Would definitely download 10/10

This application help you to set redirection email when you have drawer with different subject and you want to create a ticket on CRM ! It was very helpful on Shopify to find this. Support team are very great ! They help us a lot and they are very friendly ;)

I downloaded the app to upgrade my customer registration list and the app fulfils almost all my needs. Customer support was very helpful and assisted me throughout the whole setting up process.

We wanted to customise some of the field inputs on our form and contacted the app developer to see if it was possible. They responded really quickly and were able to action all our requests. The app functions really well and the level of service is top quality. Highly recommend.
