It's a pretty good app. You get a lot of customization features, and the conditional logic is the best way to handle customer inquiries. The support is relatively responsive and they can help with all of your needs. Would definitely download 10/10

This application help you to set redirection email when you have drawer with different subject and you want to create a ticket on CRM ! It was very helpful on Shopify to find this. Support team are very great ! They help us a lot and they are very friendly ;)

I downloaded the app to upgrade my customer registration list and the app fulfils almost all my needs. Customer support was very helpful and assisted me throughout the whole setting up process.

Very useful app especially with the payment option. Support was also helpful. Would be great if they eventually added it's own custom email blast feature so you don't have to rely on another app for emails.

Have only recently started to play around with the app to get an understanding of the forms. Had an issue with Conditional Logic which was quickly fixed by the support staff. So far so good! Looking forward to designing and implementing some of my creations.

My forms keep disappearing after they are working while for no reason. Very annoying and costly. Wish they would fix it

Been using this app across a few of our stores now for a couple of years and have found it very reliable indeed. Needed some support this weekend on a new store we are putting up and didn't expect an answer until Monday and was pleasantly surprised that we got an answer to our needs over the weekend. Great support and a great simple to use app that does what is is supposed to do

I have tried several registration form apps, and this is the best one by far! The forms are simple to create and customize, and the UI is easy to navigate. There are also a bunch of different fields you can add - I was even able to add multiple file uploads in one form which I wasn't able to do on any of the other apps I tried. Their support is also very responsive and helpful!

Would highly recommend this app if you're looking to create custom forms with customer account integration! Customer support is also great, they were able to resolve my issue fairly quickly.

Great Form builder app, one of the only ones I could find that would allow video submission which I needed for my product warranty support form. Also the dev worked with me to modify the plan to allow higher MB upload limit to more fit my shop. Great app
