Been using this app across a few of our stores now for a couple of years and have found it very reliable indeed. Needed some support this weekend on a new store we are putting up and didn't expect an answer until Monday and was pleasantly surprised that we got an answer to our needs over the weekend. Great support and a great simple to use app that does what is is supposed to do

Great Form builder app, one of the only ones I could find that would allow video submission which I needed for my product warranty support form. Also the dev worked with me to modify the plan to allow higher MB upload limit to more fit my shop. Great app

I am looking to set up a multi-step form and this one turns out to be the best. I've spent days searching only to find that no other Apps support the Page Break feature. Since the fields are way too many (with image demonstration), it would be nice to divide them into different sections and loads at a later stage. Room for improvement: 1) add the padding setting. For now, I have to manually create the tag for some spacing, or the submit button would stick to the Pre-Next bar. 2) Loading speed The shorter code generated requires at least 8 seconds until the whole content shows up. Maybe that's because the form has too many fields as I said before. And it takes time to connect to this APP as well. So I decided to copy the complete HTML code on the dashboard page. Now it loads much faster. Overall I am very satisfied with this App and would recommend to anyone who needs a multi-step questionnaire. Besides, customer support is very responsive as well, almost the same-day reply.

We installed this form to help us collect additional information for approval of wholesale accounts. We like that we are able to review the entries submitted via the form and approve/deny the entries before they can be created in our Shopify system itself.

The thought put into the variety of possible forms is awesome. Easy to create forms. After I created my form I needed assistance in setting it up on a specific page on my site. Great app and Great service. A majority of my business comes from forms and this one is a keeper. I like the conditional logic that shows certain fields based on answers in other fields. I also like the auto emails it will send upon submission of the forms, to the customer and to me. You can send them from your own email and you can customize the emails with form entries for added personalization. The support has been there to help me through a few issues as well!

Nice form, easy to use. One thing is wish the form has easy sending from option, instead of emailing the developer to activate it. Also, if the option to change the size of each fill out area was built in, it would be perfect. If there were too many info, makes the form long ex: current layout first name- one line, last name one line, if this was made where both were on the same line to save space. Customer service is great, they reply pretty quickly (within 1 day)
