I can't get any help with my form. I have reached out to live chat with no response except, let me check. I've emailed and nothing. I would like to use the conditional email option and the show/hide logic option. I've tried everything and just need some guidance......NOTHING. Don't waste your time on this tool. PS a 2 day trial??

Very good app with lots of form building functionality. Customer Service team is very responsive, my queries are always handled very swiftly. Highly recommend.

The team did a great job on the app. It works well and is simple yet effective. We needed a submission counter included and within days the team built it as a feature on their app. So we re happy and plan to use it on a few more of our sites.

The thought put into the variety of possible forms is awesome. Easy to create forms. After I created my form I needed assistance in setting it up on a specific page on my site. Got an immediate response and the team went above and beyond to quickly do what I need without me even having to ask. Was very very very impressed with the level of customer service. Great app and Great service. A majority of my business comes from forms and this one is a keeper. I like the conditional logic that shows certain fields based on answers in other fields. I also like the auto emails it will send upon submission of the forms, to the customer and to me. You can send them from your own email and you can customize the emails with form entries for added personalization. The support has been there to help me through a few issues as well!

Nice form, easy to use. One thing is wish the form has easy sending from option, instead of emailing the developer to activate it. Also, if the option to change the size of each fill out area was built in, it would be perfect. If there were too many info, makes the form long ex: current layout first name- one line, last name one line, if this was made where both were on the same line to save space. Customer service is great, they reply pretty quickly (within 1 day)

App is really helpful - but could do with some user interface upgrades to make it easier to use. We built a wholesale application form with it. There were some bugs, but the developers fixed them quickly and efficiently.

We have been using this App for two of our stores and we are really happy with it. If anything needs to be changed because of doesn't work properly or something else, the developers will do it for you.

Installed this app so that we could accept registrations from the US and Australia. Initially the app was not functioning as we had wished, but a quick message and literally anything and everything we wanted to tweak was done in a timely manner - allowing us to charge in AUD and USD. The app looks fantastic in desktop and mobile versions, interface is slick and has multiple features to adjust the design. Highly recommended - keep up the good work team.

I used this app but then noticed they only allow you to have 2 forms without upgrading to their ridiculous price plans (its a form builder people these are not expensive). Thanks but no thanks. Save your money for important things like bringing customers to your store.

We use this app to gather product registrations. It's easy to use and comes with a lot of great features. And when I wanted to tweak something the form doesn't let me do, I reached out to support and they did it for us in a jiffy. Thanks!

Super unhappy with Form Builder for several reasons: 1) No ability to duplicate a form, but instead must build each from scratch, a ridiculous waste of time. 2) Adding photos to a form has to be done within the HTML code of the page on which the form will live and frequently, any addition to the code creates errors within the data collected through the form. 3) Form data does is not connected to anything within Shopify -- new records or orders are not created within my Shopify store, requiring manual effort to create orders when customers submit via the forms. 4) "Edit Preferences" from "View Details" in my Shopify app list generates a "file not found" error, making it impossible for me to change my account preferences, cancel my account, etc. 5) Support limited to email only for first 2 levels of account, which honestly is too little for such a manually oriented app.

Excellent app and even better customer service! The conditional logic is an awesome feature and pretty easy to use once you get the hang of it. Developer's responses to my questions were prompt and professional and a couple of bugs I had with one form were resolved quickly! Highly recommend!
