It's basic but gets the job done. I used for a custom order form and had the choise between listing everything in one section, or within sections and conditional display.

Easy to use app that makes powerful forms for my business's custom orders! I was able to make great forms with the free version but the perks of the paid plan were too good to pass up.

Not Free - Do not pay for anything provided by POWr!!! After you start using the app they will inform you that you only get 25 form submissions per month for free and it will cost $11.99 for up to 100. I proceeded and eventually found another app which was actually free and uninstalled this App. After a while I realised this company was still billing me every month. To stop being billed I had to contact Shopify Support and undergo a week long process to stop being charged. When I asked for a refund for the time the app was uninstalled the developers said "As per our refund policy, we, unfortunately, cannot issue refunds". Terrible company, I paid for the app for almost 2 years and they wouldn't refund the small amount for their impossible unsubscribe process.

Versatile, fun, user-friendly! I enjoy using this app and it's one of my favorite plug-ins. You can customize the form for a nearly endless amount of uses, and although there might be a paid version somewhere I have yet to run into any limitations on the free version! The one thing I wish they did better was that there is a 5-6 second load time to boot the form onto the page, but as I put it at the bottom so people scroll down to it the damage is minimal. This might also not be a problem on other computers/browsers. Overall a great app! =^..^=
