We use this app for our online forms and it's easy to set use. The tech support is helpful and quick, although the time zone difference delays things more than we'd like. We're in the US.

useful so far although its a bit annoying when using on mobile - doesnt seem very responsive - also it could do with more options like time slots for appointments etc

This app has good basics, but for our purposes there are not nearly enough design customizations available.

Helen was very helpful in sorting my issue with install and so far so good. Would recommend as it's better than other apps I've used for my Shopify store in the past. I'm based in Northern Ireland!
