Simple product that works well. You'll need to set up your discounts on Shopify after you install the app. There response times for customer service tickets are impressive.

This is an intuitive, powerful form-building app that integrates seamlessly with Shopify. If you want to intake customer groups/types or garner feedback on your offering with simplicity, I recommend this app. Additionally, the customer support is excellent. Thanks to Emma for your help:)

We were able to customize the forms based on our key requirements, thanks to the intuitive and versatile form builder. This has greatly improved the efficiency of our customer support department, and we’re very satisfied with the results. Great, easy to use product and fast support. Had an issue with one of my forms (my own fault) and Emma had it fixed and back up and functioning in no time.
