App installed a few days ago and seems everything is working well! I was looking for something specific and the developer's team realized a perfect and fast work for my website, really happy about this app! Update: time goes on but assistance team is always great! I've updated my website theme and they make their app fit perfectly, thank you!

This APP is really fantastic, it gave me the opportunity to create 2 different registration forms, one for the private client and one for the companies. Perfect, very flexible and highly recommended. Absolutely fantastic and powerful. We had wanted to setup a private wholesale customer signup form and this made it easy breezy. Great app and highly recommended to anyone that needs to setup forms on their shopify store for any reason.

This app is great! It allows us to place a registration form on our login page so perspective customers can apply for an account with us. The info gets sent to us in a email and automatically sets them up as a customer in our shopify store. I had an issue with the setup but Sarah was very helpful and got everything fixed quickly for us. Very pleased with this app and highly recommend!

I used many form applications on different CMS's but this one is really great and easy to use. It has all the functionalities that you need to create great forms and the support of the developer is second to none. Thank you.
