The perfect upgrade for Shopify's inflexible contact form. Super easy to set up, responsive layout. Custom fields and template are a huge plus. Highly recommend!

By far the best and most robust Form builder app on Shopify. I have tried them all and this one has the most features and was the easiest to use and setup. I had a minor support issue and it was handled quickly and efficiently. Five Stars Plus.

This app does exactly what we want. We can customise the form to have the fields we want. They are multiple options to choose from. We can also customise both the customer and admin email. The customer service is fast and friendly and get any issues solved quickly.

A huge compliment for the app as well as for support! Some months ago we installed the app succesfully and we created several forms with different functionalities and layouts. The app is really easy to use and really everything is possible. A few days ago we changed something and not all forms worked as before. We couldn't find the cause and we contacted support. Although it was not was not easy to solve, they fixed it for us. Their patience and customer support is amazing!

Users can't submit forms. We tried a test submission and it doesn't submit for some reason. Horrible App.

One of the best apps you could find on shopify which works exactly as per your requirement, very customizable and precise. GREAT support team Their support team is way better compared to the platform's support team, they're always ready to provide help in any possible way, They helped me out when Shopify told me "it's beyond their capabilities" which is ridiculous honestly.

Hard to find a powerful app that is plug and play for contact forms with out hiring a developer. Installed it and configured it and it is working great.

This app is perfect for our customer application form. Customer information is copied to Shopify where we can verify their information and send them their invite. The forms are easy to setup and come with great customization features.

The form is easy to create and customise, I had some trouble setting up the google recaptcha at first, but thanks to the excellent support I was able to set it up smoothly.

Great app! Easy to install, had much more control on how the submissions get formatted, which makes replying take much less time. Hoping they add validation for email and phone soon!

We've had a few issues here and there with the plugin, but the support is so excellent each time they've been resolved within days of reporting and the developers are very responsive. Other than that the plugin has some excellent features like dynamic fields that you can alter based on user input. highly recommended.
