So far, so good! I just used this app to set up a sample request form in about 20 minutes. For a free app, this thing can do pretty much anything. I absolutely love the Mailchimp integration - exactly what I needed! I would love the functionality to be able to subscribe these users to a specific segment on my Mailchimp list. But, super happy with this app so far!

I did this app to help me create a form for a potential job applicant. it had all the fields I needed. But I wished it had the feature to add a sliding scale for getting feedback. I really liked how it was really easy to add fields for the form I was creating. I would have liked it better if it had some options to add music in the background.

Can be clunky to add to your site at times. Other than that this is a great app for creating forms - the recaptcha functionality makes things a lot easier.

The app is very simple and functional, good for now. You can play with fields and add them to your website very easily.

SO EASY to incorporate and send me an email immediately after a form has been submitted!! I am using it for my ambassador program application and it works perfectly!

I used this to build out a custom contact form. The process was super easy and everything was very intuitive. I like all of the preset options and templates.

Just set up our contact form allowing customers the opportunity to fill out a simple form requesting a quote for custom umbrellas. The settings were so simple and quick to set up and I was able to send notifications once the form is filled out to multiple email addresses so we can follow up quickly. Form Builder is off to a GREAT start! Thanks, Jim

Very easy to use and exactly what i needed! It has multiple custom templates or you can build your own and choose what pages you want it on!
