great easy and simple app, i needed it for custom products and made to order essential oils for a specific product page and it works great

Frustrating, customer service impossible to reach. Had to redo form 3 times and it would not publish or save and wasted so much of my time. I do not recommend this app at all!

I've used the app to add a contact page to my site which lets the user select which department they would like to contact.. In the section where you select whether a field is required or not, the resulting layout adds a red asterisk, it would be note to be able to adjust the colour of this. Otherwise an an amazingly simple app, great work!

great app, they really seem to understand what is needed. wish they wouldn't limit form submissions tho.

App works great, but one of those cheapskate outfits that make it difficult to unsubscribe. Unlike other good apps where when you uninstall them, they unsubscribe you automatically, you have to log into a separate account page. I wasted 2 months of subscriptions cos they only responded after multiple emails from Shopify. So 1 star perhaps harsh as app is good, but when a company costs you money because they are non responsive then I'm paying for their poor service ethic and I have zero tolerance for that.

Great app, easy to use and great for anything form related. I would suggest to maybe consider more free fancy features in design.

Nice and straight forward process! Simple and very customizable! It took me about 25 minutes to get it set up to my liking! I would recommend it.

So simple and rapid to install! I have some technical experience, but this was just so straight forward to put together and get on a Shopify page its crazy!

Good app. Really easy to install and get set up. I had a booking form for my bike service business up and running in just a few minutes. Really flexible options and extremely intuitive to use. Only two things I would have liked to see: 1) It would have been handy if I could limit the date & time options to keep them within my businesses working hours. At the moment with this form it would be possible for someone to book a bike in for 11:30pm on a Saturday night (when I am most definitely not open for business) 2) There are a few fields that have the option of being set at 'half-width'. But if I do that, it would be good if there was also a way of setting the next form element to sit next to it (i.e in a two column grid).

Very easy to use. I've completed my form just in seconds! Good job by developer. Look forward to use other apps from this developer.
