This app is great! I needed a form so I could get information from customers for a custom product. It was really easy to setup and get up and running. It integrated seamlessly into the product page with the automatic install. I especially loved that I could customize an email for the customer to receive after submission, that was a really great touch. I'm a big fan of the services this app provides, especially if needed as a simple plug in. The customer support is super helpful as well and responds very fast, from what I have experienced.

Super easy to use - made a form in less than 10 minutes. Don't love that support is only via email and seems to be on European timing. Otherwise, great!

I used this app to install a custom order form section in my web store. It was easy to use, and maybe I missed it, but I would like to be able to add pictures to the custom form. Overall, great, and I will use to create again!

They have a huge problem with the contact fields on Ipads and Cellphones = it don’t work! We have contact them but they don’t answer...well at first, but after long time and still problem - they have desided to not answer. This problem occur on all cellphones/Ipads. The fields is not possible to fill. Nothing happen when you click on them. 1 star is to high - because this app do not work - and the customer "service" do not exist. So, you all people that gave this app 5 stars...have you checked the function in a mobile or Ipad? If not do that - and you will uppdate your review to 1 star.
