Outstanding customer service from Richcarde today! I was having trouble with a particular app that I had purchased through Shopify. I had a Shopify Partner working to integrate the app on my site but was running into issues getting it to work correctly. He listened to my explanation and was able to solve my problem right away. I’m disappointed that the form doesn’t support custom CSS. It really limits the flexibility.

App works well. I use this as a form for my customers to submit UGC to be featured on our site + socials. Will change to 5 stars when the multiple file upload feature works properly. I'm currently using four upload fields to allow users to upload multiple photos, and the paid plan is required to do this.
Clever app.. easy to use (especially after watching some youtube videos) and great support online if you have any questions. I created a newsletter sign up form with a free guide/checklist.
I have had terrible problems with the app and there is no response from the support team after many days. Don't waste you money.
Loads very slowly. Almost unusable with edge browser. Contacted support several times and no luck fixing
Customer support is excellent. Fast to respond, thorough, and thoughtful. They brought up suggestions to improve my app experience that I hadn't even considered. Forms are easy to create and navigate.
